Students registered in the programs listed below must complete one of the linked medical forms and submit it to the Campus Health Centre by August 7, 2009.

Provincial regulations require students in the following programs to have the form completed. Immunization records must be up-to-date and complete. Failure to submit a completed health form can result in your withdrawal from the program.

When you submit your immunization requirements, you must enclose a cheque or money order in the amount of $10.00 payable to Canadore College Campus Health Centre for reviewing your immunization requirements. When the requirements have been complete, you will be issued a Clearance Card to present to your faculty.
